Item 1.04
Mine Safety  Reporting of Shutdowns and Patterns of Violations.
(a) On June 27, 2019, a subsidiary of U.S. Silica Holdings, Inc. (the "Company") received an order issued under section 107(a) ("the Order") of the Federal Mine Safety and Health Act, related to improper use of safety equipment by a miner at the Colado Plant (Mine ID 26-00680) near Lovelock, Nevada. The miner was ordered to withdraw from the area to receive additional training in the use of the equipment. The miner received such training and the Order was terminated on the same day. No one was injured as a result of the condition alleged in the Order. The Company is reviewing the Order and may seek judicial review of its issuance.
